RI Residents who will be 15 years of age:
Step 1: Download the “Intent to Employ Minor” Form (Download Form Here)
Step 2: Check the 14-15 Years of Age box
Step 3: Complete Items 1 & 2
Step 4: Parent/Guardian sign (in ink) Item 10 and complete Item 11
Step 5: Scan and email the form to [email protected] . We will sign the form and complete Items 8 & 9 and send the form back to you.
Step 6: Bring completed form along with proof of age such as a copy of your Birth Certificate, Baptismal Certificate, Passport, or School Records to the School Department in the city/town where you live, and they will issue you either a Special Limited Permit to Work (The Working Papers)
Step 7: Upload the Special Limited Permit to Work to your account in WorkBright with your other pre-employment forms

Check with your Town’s school department to see if they will complete the RI Special Limited Permit to Work (Found here) or
Bring completed Intent to Employ Minor form along with proof of age such as a copy of your Birth Certificate, Baptismal Certificate, Passport, or School Records in person to: Rhode Island Department of Labor and Training (RIDLT Workplace Fraud Unit, 1511 Pontiac Ave., Bldg.#70-2, Cranston, RI 02920.
•The office is open Monday- Friday 8:30 am- 4:00 pm. The office will not be open on June 19th.
• You must contact Katie Hall at the RIDLT to make an appointment. Katie can be reached at 401-462-8583 or [email protected]
• Prior to your appointment, you must complete the Intent to Employ Minor Form and Special Limited Permit to WorK. Do not sign them! You will be required to sign in person at the RIDLT office
•Staff may not report for work until these forms are completed and signed by the RIDLT
